Laurel County Sheriff John Root says an investigation of the crash revealed a car driven by Gloria Faye Engle, 72, of London pulled out from Sinking Creek Road in front of a pickup truck driven by Chad Brent Sturgill, 30 who is also of London.
Deputies say after the vehicles hit one another they left the road and ended up off the right-hand shoulder of the roadway over an embankment.
Inside the car with Engle were her eight-year-old great-granddaughter and her 14-year-old great-grandson. Deputies say Engle and both children were trapped inside the car and had to be extricated from the vehicle.
All three were pronounced dead at the scene by Laurel County Coroner Doug Bowling.
Sheriff Root says Sturgill was not injured in the crash.
Investigators are looking into whether the bright morning sun was possibly a contributing factor.

They tell WYMT preliminary investigation shows a car pulled out in front of a pickup truck.
All 3 passengers in the car have been pronounced dead.
The one person in the truck was taken to Saint Josephs Hospital in London.
Officials say they expect the westbound lane of KY 80 will be closed for four hours as they reconstruct the scene.
